Please complete the following International Student Services forms as soon as possible:

You should have already received physical copies of or links to the following forms from various other offices. Please refer to this checklist [PDF] when preparing / mailing the documents.

StART forms should be returned to International Student Services. Other forms listed below can be returned to either the corresponding offices or International Student Services.

• Required Medical Forms (Student Health Services)
• Student Housing Application (online) & Personal Data Sheet (Residential Life)
• StART Packet (including course registration guide, course checklist, advising information, etc) (StART)
• Account Agreement and Statement of Financial Responsibility (Student Accounts)
